This was set up to decrease the stress and time spent at the Doctor’s office for the first time.


The forms below will decrease the amount of time spent for your first visit and may be completed in the comfort of your own home at your convenience and if so desired, faxed to our office ahead of time. There is no requirement to fill these forms out ahead of time or fax them ahead of time, you may simply bring them with you, or they will be provided to you at the time of your appointment.

  • Health History Form: This gives us a picture of your past and present health status, gives us a glimpse into your family’s health history as it pertains to you, and is a simple and effective way to start the process of increasing your overall health status.
  • Notice of Privacy Practices Form: This form is for the protection of your personal health information and details your rights.

What insurance plans do you accept?

Most plans are accepted at our office. We accept auto accident, workers compensation (on the job injury), personal injury, and many insurance carriers. Please call to find out if your insurance plan covers, since there are so many variations of plans. Our staff will explain your benefits before initiating care at no charge.

How do I schedule an appointment?

You can schedule an appointment by calling our office during business hours, using our online scheduling system on our website, or through our patient portal if you are an existing patient.

What should I bring to my first appointment?

Please bring a Photo ID, Insurance Card, and Preferred payment method (cash, check, VISA, Mastercard).

What if I don't have insurance?

If you have little to no chiropractic insurance coverage, flexible payment options can be arranged. There is still a way for you to receive the care you need. Chiropractic is extremely cost effective and helps you reach your health goals quickly.
